Sports Authorization Form

I hereby authorize my son/daughter to take part in the Physical Education and Sports programme activities organized by the school within school grounds or any other location chosen for such purpose.  I do not hold the school responsible for any incident which affects my son/daughter while participating, as long as all rules established within the regulations of the activity have been observed and there exists no fault, negligence or recklessness in the organization and supervision on the school’s part.
